초록: 

한국의 약물중독 치료는 교정기관 중심의 강제치료가 주를 이루고 있다. 치료공동체는상호자조를 통해 변화를 시도하는 모형으로 오랜 기간 효과적인 중독치료모형으로 제안되어왔다. 본 연구는 약물중독 치료공동체에 대해 고찰하고 약물사용자의 치료공동체서비스이용 의사결정과정과 그 영향요인을 분석하는 데 목적이 있다. 연구는 요인모형으로 대표되는 건강서비스이용행동모형과 과정모형인 의사결정과정모형을 통합한 도움추구과정단계모형을 분석틀로 하였다. 분석결과 약물사용자의 치료공동체 서비스이용의사결정과정은 문제인식-치료서비스 이용의도-치료공동체 서비스이용 의사결정의 단계를 거치는 것으로 입증되었다. 구체적으로 살펴보면, 성별, 과거치료경험, 유죄판결횟수, 사회적지지, 약물사회망, 약물남용심각도는 문제인식 단계에 영향을 미치며, 그중성별과 과거치료경험, 약물남용심각도는 문제인식단계를 거쳐 치료서비스 이용의도에영향을 미치는 것으로 나타났다. 치료공동체 서비스이용 의사결정까지 유의미한 영향을미치는 요인은 과거치료경험으로 분석되었다.

This study aims to review Therapeutic Community(TC) Model which has been proved as effective drug abuse treatment and to analyze factors influencing the decision-making process of TC service among drug users. This study was conducted based on the help-seeking process model which integrated the factor model of health service utilization and the process model of decision-making process model. Findings showed drug users' decision to use the TC service has undergone the stage of problem recognition – intention to use of treatment services – decision-making of TC service use. In specific, gender, previous treatment experience, number of convictions, social support, drug social network, and severity of drug use influenced the first stage. Next, gender, previous treatment experience, and severity of drug use affected the second stage through the first one. Finally, previous treatment experience was the only significant factor in the decision-making process for the use of TC Service.
