초록: 

가족은 정신장애인 당사자의 치료 및 신상과 관련한 다양한 의사결정과정에서 대리인 역할을 해 오고 있다. 만약 정신장애인의 의사가 가족의 의사와 유사하다면 가족은 가장 적절한 의사결정대리인이 될 수 있다. 하지만 아직까지 가족의 의사가 정신장애인 당사자의 의사와 동일한지에 대한 경험적 연구는 제한적이며 가족과 정신장애인 당사자의 정신요양시설에 대한 인지적 평가가 지역사회치료 결정에 어떤 영향을 주는지에 대한 연구도 거의 없다. 이러한 점을 고려하여 본 연구는 정신장애인 당사자와 가족의 정신요양시설에 대한 평가와 지역사회서비스 선호도가 유사한지에 대해 정신장애인 당사자 1,280명과 가족 379명을 대상으로 구조방정식을 통해 검증하였다. 분석결과 가족이 정신장애인 당사자 보다 정신요양시설을 긍정적으로 평가하고, 긍정적으로 정신요양시설을 평가할수록 지역치료선호도가 떨어지는 것으로 나타났다. 결과적으로 가족이 정신장애인 당사자에 비해 지역치료를 덜 선호하는 것으로 나타났으며 이는 정신장애인 당사자와 가족의 의사는 동일하지 않다는 것을 보여준다. 이러한 연구결과를 바탕으로 현재 가족중심의 의사결정대리의 문제점과 의사결정지원제도의 향후 발전방향에 대해서 논의하였다.

Family of individuals with MI have been involved in various decision making processes for their family members with MI. It is acceptable if decision of family members is coincide with that of individuals with MI. Few empirical research studies, however, have examined whether decisions of family members are coincide with that of individuals with MI. This study aims to examine if mental health facility evaluation and community service preference of family are equivalent with those of individuals with MI using structural equation modeling(SEM). Sample consists of 1,280 individuals with MI and 379 family members who participated in the Survey of Long-term Care Facility(LTCF). Major results are as follows. Family evaluated mental health facility environment more positively and were less likely to prefer community services, indicating that family’s decision is not equal to that of individuals with MI. In conclusion, these results indicates that decision of family is not equal to that of individuals with MI. Based on these results, this study discussed the future directions of decision making support for individual with MI.
